Position Summary
The Community Health Nurse (CHN) will supervise and coordinate all activities of nursing personnel; serve as liaison between
clinical staff and administrative personnel. The CHN works with community stakeholders to improve the health and wellbeing of
the population in our community. The CHN collaborates with health and clinical services to use knowledge of healthy growth and
development, healthy behaviors, community wellness, infection diseases, evidence informed practices and the assessment of the
community to plan health promotion, prevention initiatives and essential services. Programs and services will be delivered using
relevant guidelines and protocols in clinical health care services based on practical, scientifically sound and socially accepted
methods and technology to members of the GRFN Community while functioning within legislative requirements, regulations,
Standards of Practice set out by the Colleges of Nurses, and the health services policies and procedures and the Mission, Beliefs
and Vision of the Garden
